Just e-mail Les (leswrap@cs.com) six things:
(1) your name (as it appears on your credit card),
(2) your credit card number,
(3) the expiration date of the card,
(4) indicate if your card is Visa or Mastercard,
(5) provide your complete address and
(6) your phone number (in case there are any problems and he needs to call you back).
